I paid for Trend Antivirus in October but I noticed that our Trend icon is missing on our desktop. Do we have to download every time we renew our subscription.

First, here's an easy way to check if your Antivirus+ Security still works on the computer:

The default location of Trend Micro Security is as followed.
In Windows Start Menu:
Start - All Programs - Trend Micro Antivirus+ Security

In Windows Explorer: 
C:\Program Files\Trend Micro

If you cannot find the Trend Micro Antivirus+ Security icon on the desktop, we have a support article to help you restore the missing desktop shortcut of  Antivirus+ Security:

If you cannot find Antivirus+ Security's icon in system tray or in Windows Start menu, we assume that the program is damaged somehow. Please follow solution that Tom has provided to reinstall Antivirus+ Security:
