having issues trying to gert my cashback offer

when I fill in the fields for the cashback offer it asks for a code not provided on my docket. I made a instore purchase not a online purchase so this may have created an issue

Purchases with cashback from the retail store can be claimed on this website:

The cashback coupon code isn't present in that form since it's not a required. Please submit a claim on the said link, you will receive an email afterward which has the eticket ID.
